Informed Consent and Acknowledgement:
I hereby give my approval for my child’s participation in all activities prepared by Lennox Lewis League of Champions Foundation (LOCF) during the selected camp. In exchange for the acceptance of said child’s candidacy by LOCF., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless LOCF and all its respective officers, agents, and representatives from all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.
In case of injury to said child, I hereby waive all claims against LOCF. including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports and recreational activities.
Photo and Video Disclaimer:
The Lennox Lewis League of Champions Foundation (LOCF) reserves the right to use any photograph/video taken at any event sponsored by LOCF, without the expressed written permission of those included within the photograph/video. LOCF may use the photograph/video in publications or other media material produced, used or contracted by LOCF including but not limited to: brochures, invitations, books, newspapers, magazines, television, websites, etc.
A person attending a LOCF event who does not wish to have their image recorded for distribution should make their wishes known to the photographer, and/or the event organizers, and/or contact LOCF at contact@locfoundation.org in writing of his/her intentions and include a photograph. By participating in a LOCF event or by failing to notify LOCF, in writing, your desire to not have your photograph used by LOCF, you are agreeing to release, defend, hold harmless and indemnify LOCF from any and all claims involving the use of your picture or likeness.
Medical Release and Authorization:
I/we the parents/guardian of the above participant give my/our permission to participate in any activities & events. I/we know that participation in boxing may result in serious injury & we waive, release, absolve, indemnify, and agree to hold the Lennox Lewis League of Champions Foundation, Inc. and the organizers, board members, sponsors, supervisors & participants for any claim arising out of injury to my/our child whether the result of negligence or any other causes.
It is understood that these activities involve an element of risk of accidents & understanding of these risks. I/we hereby accept and assume those risks. It is further agreed that this waiver, release & assumption of risk is to be binding on my heirs & assigns. I will also abide by all the organization’s rules.
By signing, I also agree to follow the Lennox Lewis League of Champions Foundation conduct. I/we acknowledge that the participant’s age and grade level information are factual and accurate.
Failure to follow the rules may result in my child’s or my dismissal from the camp indefinitely.
As Parent and/or Guardian of the named athlete or camper, I hereby authorize the treatment by a qualified and licensed medical professional, of the minor child in the event of an emergency. Permission is also granted to the LOCF Organizers and its affiliates including Directors, Coaches, and Team Parents to provide minor treatment application ointments and Band-Aid if needed.
Release authorized on the dates and/or duration of the registered camp season.
This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under needed circumstances, for my child, in my absence.
